Instructions for Out-of-Town Patients

Breasts:

            Front and Side view of chest with no clothing in the photograph. Views to include area from neck to belly button.

Tummy:

            Front and Side view of abdomen with no clothing in the photograph. Views to include area below breasts and
           above pubic bone.

Face:

            Front view, Both sides, and ¾ view. Chin level, hair out of face; include area from top of head to collar bone.

Legs:

            Front, Both sides, and back views. Please wear panties. Include area from hip bones to ankles.
